新台灣仲介公司註冊時所需要的文件

·       填寫菲勞中心線上網路申請表格(LBR 10-B)-下載。

·       提供仲介公司業務代表的相關資訊，填寫台灣人力仲介公司資料更新申請表格(LBR 11-B)-下載。

·       附上以下文件

-      POEA申請表格(LBR 12-B)-下載-2份。

-      勞委會許可證影本2份。

-      負責人的身份證及護照影本-各2份。

-      公司執照中文及英文影本，並須經過法院公證-1份

-      業務代表的身份證影本-1份。

·         提交所要求的文件與新台幣1,435元整。

·         於三個工作天之後可以取回驗證過的外勞申請文件及線上網路申請系統密碼。

更新台灣仲介公司執照所需要的文件

·       填寫菲勞中心線上網路申請表格(LBR 10-B)-下載。

·       附上以下文件

-        POEA申請表格(LBR 12-B)-下載 -2份 。

-        負責人的身份證及護照影本-各2份。

-        勞委會許可證影本2份。

·         提交所要求的文件與新台幣1,435元整。

更新台灣仲介公司相關資訊時所需要的文件

·       填寫台灣人力仲介公司資料線上網路更新申請表格-下載-1份。

·       附上以下文件

-      POEA申請表格 [LBR 12-B] -下載-2份。

-      勞委會許可證影本2份。

-      負責人的身份證及護照影本-各2份。

-      公司執照中文及英文影本，並須經過法院公證-1份

·              提交所要求的文件與新台幣1,435元整。

增加或變更業務代表時所需要的文件

·        填寫台灣人力仲介公司資料更新申請表格(LBR 11-B)-下載-1份。

·        業務代表的身份證影本-1份。

·        提交出相關文件進行資訊更新，免收費用。

台北辦事處上班時間

送件時間：上午九點至十一點三十分。

領件時間：上午九點至十一點三十分＆下午一點至三點三十分。             

其他業務處理時間：上午九點至十一點三十分＆下午一點至三點三十分。

Registration & Information update Requirements

Registration of New TMA

·       Accomplish the MLC Online System Application Form (LBR 10-B) – download

·       Provide Liaison Officer Information.  Accomplish the Information Update Application Form for TMA (LBR 11-B) – download

·       Attach the following

-      POEA Registration form (LBR 12-B) 2x– download

-      Photocopy of CLA Permit 2x

-      President’s ID and passport copy 2x

-      Photocopy of company license with English translation, notarized by the District Court 

-      Liaison Officer’s ID copy

·         Submit the requirements and pay Verification fee NT$1,435

·         Claim your documents and online system password after 3 working days.

Registration of TMA License Renewal

·       Accomplish the MLC Online System Application Form (LBR 10-B) – download

·       Attach the following

-        POEA Registration form (LBR 12-B)– download

-        President’s ID and passport copy

-        Photocopy of CLA Permit

·         Submit the requirements and pay Verification fee NT$1,435.

·         Claim your documents after 3 working days.

Change of TMA Information

·        Accomplish the Information Update Application Form for TMA – download

·       Attach the following

-      POEA Registration form [LBR 12-B] 2x– download

-      Photocopy of CLA Permit 2x

-      President’s ID and passport copy 2x

-      Photocopy of company license with English translation, notarized by the District Court     

·               Submit the requirements and pay Verification fee NT$1,435.

Addition or Change in Liaison Officers

·        Accomplish the Information Update Application Form for TMA (LBR 11-B) – download

·        Submit the requirements, free of charge.
